plone.app.upgrade 1.1.1 was missing the forward-porting of some upgrade 
steps from recent versions of p.a.upgrade 1.0.x, with the result that 
upgrades from Plone 4.0.7 would fail. I just released 1.1.2 to fix this 
issue; please include it in 4.1.1 final.
